Cleveland ISD has updated the Senate Bill 12 parent form based on new guidance from the Texas Education Agency. The form now includes additional sections for health services, counseling, and student clubs.

Even if you previously completed this form, we ask that you fill out the updated version to ensure your child continues to have access to all available services and care.
